A Non-Substantive Program Modification form must be submitted and approved prior to implementation, for any of the following revisions to currently licensed programs:
1. A change in the program name,
2. Tuition and/or fess increases (the total of tuition and fees must be provided),
3. A change of less than 25% in the clock hours, credit hours, curriculum content or program length,
4. A change in the mode of delivery of the program.
Form submission: the Non-Substantive Program Modification for must be signed by a duly authorized representative of the institution. If the revisions are for several campuses, one form amy be used, but all locations must be properly identified. Staff will process the changes and return the form signed and approved.
